Register with our practice
We are currently taking on new patients. To register with the practice you must complete the online registration form above. If you are unable to access this form online then a paper copy can be downloaded and printed.
If you require access to online services you will need to provide photo ID. Please phone and speak to Reception regarding this.
You will need to attend the surgery for a new patient screening appointment with our Health Care Assistant.
When you have completed a registration form please contact the surgery to make an appointment with the Healthcare Assistant for a New Patient Screening in order for us to finalise your registration. You will not be able to see a doctor until this form is completed and you have seen the Health Care Assistant.
Medical treatment is available from the date of registration. Please contact reception for further information.

Named GP
The Government requires GP Practices to allocate a Named Accountable GP to all their patients from April 2015.
All of our patients have now duly been allocated a Named Accountable GP but can still continue to see any GP of choice within the Practice.
If you wish to know the name of your Accountable GP please ask when you are next in contact with the Practice.
